Regular
Forms that are on the National Forms Website
Most
of the information that follows is also on the National Forms
Website. Some additional information has been added to help member
navigate the National Forms Website.
Coast
Guard Auxiliary's National Forms Site
contains the most recently released versions of
Auxiliary forms.
Many of the forms provide fill-in capability, and some include new printing
and numeric totaling features.
A
left click on the light blue form title will open
the form in a new window to fill out and print.
If you right click on
light blue form title and choose "Save Target As" on the
menu that come up, you can save the form to your computer to use as often as you
want. It should be noted that form are changed often and by saving it to
you computer you could run into a situation were you are using a from with an
outdated revision number. It would be better to open the form by a left
click then save it to your Favorites
so that you have a link to the form.

EMAIL
FORMS Are approved for use in
Division 2. SO/IS Division 2 has approved use of these by
flotilla's in Division 2. You can send them directly to SO/IS but you also need to send a
copy to FSO/IS Judy Davidson. Please read direction below before filling
out one of these forms. They will tell you how to add more than one email
address to the form.
These
forms are identical in content to normal forms, with the exception that they
allow on-line submission to whomever you like. The software requirements for
these forms to work properly are Acrobat Reader 5 and above. Your browser must
support Reader opening as a helper application while on-line; National has
stated that they cannot test
with every browser currently in the field. Adobe is aware of an incompatibility
with Netscape Navigator 6 which does not allow the forms submission feature to
work, so NN6 cannot be used at present. MSIE does not appear to have any problem
with the current version.
In
order to use these forms, you must *left* click on the form number (do NOT *right* click as you normally do to download a form!) and be patient, as
the form itself must be copied into the browser and the Reader has to start. You may
experience a white screen for a period of time- that's normal. When you have
finished filling out the form, you can print a copy of the completed form by
using the
printer icon in the Reader window
or the print button at the bottom of the form.
Once you click on the "Submit" button, the data will be sent to the
address you've specified and an acknowledgement screen will appear confirming
what was sent. NOTE: If nothing happens when you
click "SUBMIT" see the FAQ
page on the
national website.
You
may send multiple addressees your reports; simply add a comma (,) between
addresses. For example, to send to aux1@cgaux.org and coasty@isp.com, enter
"aux1@cgaux.org, coasty@isp.com" (without the quotation marks).
You need a space between the comma and the next email address for it to
work. Flotilla members can email the
FSO/CS
for help with this. He has a suggestion on how you can set this all up in
Note Pad to make it easier to add the email addresses.
If
you choose to send a copy to yourself and add your email address to send
to and have a Spam filter running or have AOL as your service provider you
will not get a copy.
The
best thing to do is print a copy before sending so that you know you have a
copy.
If
you have difficulties, please report them using the
Request
for Technical Assistance form on the national
website.
