1. To fill out the form, click on the down arrow where is says "Choose Staff Office From List" then click on your Staff Office. Your name will also be added to the report from this choice even though you don't see it. 2. Now click on the down arrow where is says "Choose Your Website Email Address" and choose the one that starts with your "FSO" designation. This will allow anyone that receive the report to reply and ask question. 3. Next go down and fill in information for the remaining 5 blocks by clicking once with in the block with your mouse cursor and deleting "No Report" then type in your report for that block. If you have nothing to fill in for a block leave it the way it is with "No Report". 4. When you are done click the "Send Now" button at the bottom of the form and it will be sent to Flotilla Commander, Vice Flotilla Commander, Immediate Past Flotilla Commander, and all Flotilla Staff Officers. 5. If you have a Flotilla Event or Member Training to report don't forget to fill out a Casco Bay Flotilla Calendar Event Or Member Training Advertisement Request Form with as much information as you can give. You can put in up to 10 dates for an Event or Member Training on the form. If you have more than 10 dates just submit another form. There is a box at the bottom of the form for comments. You can put as much information there as you want. The more information you put in the better we can advertise it for you. It will be used for the Flotilla Website Calendar Page and Member Training pages. The form is also sent to FSO/PB for the Flotilla Meeting Notice/Newsletter and the FSO/SR for flotilla records. If you find that you need to cancel something you send this form in for, please send an email to fsocspbsr.21@uscgauxsoportlandme.com, this will notify all that receive the form to cancel it. When you press the "Send Now" button at the bottom of the form, date and time of submission will be add to the report.
