Operational Support (OS) 

(This is the one everyone now needs to do.)

Direct Operational (DO) Qualifications/designations in:

 

The balance of the Auxiliary membership and their activities not listed  in DO. Personnel Security Investigation (PSI) package consists of the following forms:

a. Fingerprint Cards (FD-258) - 2 cards completely filled out, using DIRAUX as the Employer Address.

b. OFI Form 86C (Special Agreement Check)

(1) American (by birth) citizens need to fill out 1 through 6 and 13.a & 13.b
(2) Naturalized citizens need to fill out 1 through 6 and 13.a., 13.b & 3.c
(3) Dual citizens need to fill out 1 through 6 and 13.a., 13.b., 13.c & 13.d
(4) Coast Guard Security Center (SECCEN) will fill out remaining blocks.

c. Standard Form 85 - Authorization For Release of Information (page 6 only)

d.  USCG Auxiliary/SECCEN Verification of U.S. Citizenship form

Personnel Security Investigation (PSI) package is the standard National Agency Check with Law and Credit (NACLC), consisting of the following forms:

a. Questionnaire for National Security Positions (SF-86), ( 2 copies, both with original signatures.
b.  Fair Credit Reporting Act (DHS 11000-9 form (old DOT form 1631))
c.  Fingerprint Cards (FD-258), 2 cards
d.  USCG Auxiliary/SECCEN Verification of U.S. Citizenship form

Step-by-step procedures (subj to modification by local DIRAUX as needed for AOR):

1. Auxiliarist fills out appropriate PSI package based upon qualifications.
2. Package is submitted to the local DIRAUX.
3. DIRAUX submits via a quota system to the SECCEN.
4. Package received, Security Folder created, packaged scoped (quality control), if all paperwork is correct, a request is forwarded to the Office of Personnel Management (OPM) to perform the investigation.
5. Once results are returned to SECCEN, the members security folder and investigation package are married and an adjudicative determination for either suitability (OS) or eligibility based on national adjudicative standards for access to classified information (DO) is made. Clearances will not be granted, unless there is a need-to-know. Only active duty Coast Guard unit commanders using a CG-5588 (Personnel Security Action) form may make this official request.
6. Results are entered into AUXDATA. SECCEN closes investigation file and maintains control of security folder.
7. When new security determinations are entered, DIRAUX’s are prompted in AUXDATA to look at these decisions.
8. DIRAUX’s notify Auxiliarists via local procedures.

Special note:

 All DO packages must have 1 original SF-86 with signature in blue, and 1 copy of SF-86 with signature in blue.  This is a must!  The copy stays at SECCEN and the original is forwarded to OPM.

 

 

Security Forms

To Download these forms from the Flotilla website, Right Click on the form.  On the Menu that comes up click on "Save Target As".  It should save the file to your "My Documents File".

Please make sure your system has at least ADOBE ACROBAT Version 5.x.  Some of these forms are fill in.  However you do need to download as instructed above for them to work.  You should also be aware that if you only have the Adobe Acrobat Reader you can not save your work.  You will have to either complete it in one session or print out what you have done and then re-enter it again to complete the form.  Once you have completed the form you can them print out copies.

Some of the forms can be downloaded in either Adobe Acrobat or Microsoft Word.  Some of the Word versions are a fill in forms.  Use the space bar to get your entries in the correct position.  Recommend that you use your mouse and cursor to go from one block to another and NOT use the Tab Key.  Some of the Adobe forms are pen and ink fill in after you print them out.

Security Policy Directives and Announcements

The Chief Director is providing the following links to information in order to provide you with additional information concerning the Security Policy Directives, Announcements and Forms. You may download and print out any of the forms for review. However, it may be 2 years before your DIRAUX asks you to submit the SF86 form. You should not sign them, date them, nor send them to anyone until your DIRAUX promulgates procedures for your district and you are asked to submit them. When asked to submit the SF 86 form, you need to review the information you may have prepared in advance to make sure it is still current and within the guidelines of the SF 86 form requirements as to time periods.
